AUTHORIZATION: Cremation is an irreversible and final process.  By my signature, I certify that I have the authority to authorize the cremation of the named pet above and give Cherish Pet Cremation Services, LLC and its’ staff the complete authority to provide cremation services and final disposition for herein referenced pet. Fees for these services have been explained to me and I assume full responsibility for all charges applicable to such services.

RELEASE AND CERTIFICATION: I agree to release and indemnify and hold harmless Cherish Pet Cremation Service, LLCs and their owners, employees and successors from all liability, claim or expense resulting from and related to the performance of the requested services agreed in the related request for service.

FINAL DISPOSITION: Cherish Pet Cremation Services, LLC is authorized to release the cremated remains to the owner or their authorized representative following the cremation.  I understand that if no arrangements for the final disposition of the cremated remains have been made within 30 days after the cremation or if I have not taken delivery of or caused the delivery of the cremated remains, or in the event the arrangements of the final disposition have not been carried out within the 30 day period because of the inaction of a party other than Cherish Pet Cremation Services including but not limited to payment in full, then Cherish Pet Cremation Services may dispose of the cremated remains in any dignified manner, including scattering of ashes.  Additionally, I understand and acknowledge that my pet’s cremated remains will not be released to me until any balance owed is paid in full.

PET DISCLOSURES: I certify that my pet has not bitten any person or other animal during the past ten (10) days, is not suffering from rabies, and has not been exposed to any other animals suffering from rabies.

ACKNOWLEDGEMENT: To the best of my knowledge the information I have provided is accurate and complete.  I have carefully read, fully understand, and agree with the terms contained in this form.
